Normal potassium homeostasis depends on maintenance of its internal and external balance.
The primary organ for preserving external K balance is the kidney; internal factors
primarily include insulin, catecholamines, and acidosis. Patients at risk for hyperkalemia
include those with renal failure, diabetes mellitus, hyporenin- emic hypoaldosteronism,
or on a variety of drugs. The cardiac effects of severe hyperkalemia may require urgent
therapy in a critical care setting. Because hyperkalemia has many causes, an organized
approach is necessary to determine the underlying cause.
